Forms for HR
For applicants:
As an applicant, you will be asked to fill up forms requiring you to disclose and share personal information about yourself and submit documents supporting your application to First Oceanic Property Management Inc. (“FOPM”).
This information shall be shared to various internal departments and government agencies to facilitate the process of recruitment and hiring in the event of being selected. These shall form part of your 201 as employee wherein you have the right access and correct if necessary. This 201 file under your name shall be kept confidential and shall only be disclosed for legitimate and valid purpose by reason of your employment. In case you decide to resign or be dismissed from the company, your files shall be destroyed after 2 years from severance to FOPM.
However, if you failed to be selected as employee of FOPM, said documents filed and submitted shall be destroyed after 3months if you failed to be considered for other positions that is vacant within the company.
For APE:
As employee of FOPM, it is a requirement to maintain a healthy body in the performance of duties. An annually conducted physical examination shall be administered company-wide covering various parts of the body to determine potential illness and manage and heal current sicknesses. Various medical reports and findings shall be issued by the HMO provider to you and FOPM.
By affixing your signature below, you are giving your consent for FOPM thru HR and Safety and Security Department to secure a copy of the results of your physical examination. This shall be kept confidential and shall be used for monitoring of your health condition and to determine your fitness to work for the company. Once your employment is discontinued this file shall be destroyed. Thank you.

falcification/misrepresentation or ommision done by me will greatly affect my application and cause terminations hould i be employed in this company prior to the employement reference checks
